- We are seeking a proactive, detail-oriented Clinical Care Coordinator to support a modern, patient-centered medical practice that integrates in-person care, virtual care, and technology-enabled clinical workflows. This role combines traditional medical assistant responsibilities with advanced care coordination, remote patient monitoring oversight, and ongoing patient communication.
The Clinical Care Coordinator serves as a central operational and clinical support role, ensuring continuity of care across providers, specialists, digital platforms, and patient touchpoints. This position is essential to keeping clinical workflows organized, proactive, and responsive.
Key Responsibilities
Clinical & Patient Support
- Support in-person and telehealth visits, including patient intake, chart preparation, documentation support, and post-visit follow-up
- Respond to patient questions through secure messaging platforms and phone communication as directed
- Follow up on pending clinical items, including labs, imaging, referrals, results, and care plan next steps
- Maintain accurate, timely clinical documentation in the EHR
Care Coordination
- Coordinate medical care with outside specialists, imaging centers, laboratories, and pharmacies
- Track referrals, ensure receipt of consult notes and results, and escalate outstanding items to providers
- Serve as a liaison between patients, providers, and external clinical partners
Remote Patient Monitoring (RPM)
- Review and track RPM data including vitals, CGM data, blood pressure, weight, and wearable outputs
- Flag abnormal or actionable data and communicate findings to providers per protocol
- Support patient adherence to monitoring programs and device use
Orders & Prescriptions
- Prepare, send, and track lab orders, imaging orders, referrals, and prescriptions as directed by providers
- Assist with medication refills, prior authorizations, and pharmacy communications
- Ensure orders are completed, results are received, and follow-up actions are documented
Workflow & Operations
- Coordinate information across multiple systems including EHRs, RPM dashboards, telehealth platforms, and patient communication tools
- Manage task queues and ensure clinical workflows remain timely and organized
- Support providers by anticipating care needs and closing clinical loops
Compliance & Quality
- Ensure all clinical tasks, documentation, and communications comply with HIPAA, regulatory requirements, and practice protocols
- Maintain professionalism, discretion, and patient confidentiality at all times
Technology & Communication
This role requires daily use of digital health tools, including EHR systems, secure patient messaging, telehealth platforms, and remote monitoring dashboards. Strong written communication, comfort with technology, and the ability to adapt to evolving systems are essential.
Qualifications
- Associate’s degree or equivalent experience
- Strong typing, writing, and computer skills
- Excellent organization, attention to detail, and follow-through
- Ability to manage multiple tasks in a fast-paced clinical environment
- Professional communication skills with patients, providers, and external partners
- Working knowledge of HIPAA and patient confidentiality
Preferred
- Prior experience as a medical assistant, virtual medical assistant, or medical scribe
- Experience coordinating care or managing clinical workflows
- Familiarity with medical terminology and EHR systems
- Experience with remote patient monitoring or digital health platforms
- Background in healthcare, nursing, public health, pre-med, or allied health
- Interest in preventive, metabolic, or lifestyle-focused medicine

Job Type: Full-time
Pay: $49,000.00 - $55,000.00 per year
