(CNA / MA Required)

Location: In-Person (Atascadero, CA)
Reports To: Clinical Operations Supervisor
Compensation: $24.00 - $32.00 per hour
Role Type: Full-Time (40 hrs/week)

The Role

Clinical Care Coordinators are the connective tissue between our clinical teams, families, and operational systems. This role is ideal for someone who thrives in fast-paced environments, loves helping people, and can balance compassion with organization.

As a Clinical Care Coordinator, you help ensure that clients move smoothly through care — from intake to ongoing support — while creating a calm, welcoming experience for families and individuals navigating mental health services.

This role works closely with therapists, psychiatric providers, ECM teams, schools, pharmacies, and caregivers to reduce barriers and improve continuity of care.

Key Responsibilities

Client Support & Coordination

  • Serve as a primary point of contact for individuals and families via phone/ text.
  • Assist clients with onboarding paperwork, intake forms, scheduling, and care coordination
  • Provide warm, emotionally intelligent support to families and individuals who may be overwhelmed
  • Coordinate follow-up communication after appointments and missed visits

Clinical Operations Support

  • Support therapists and prescribers with scheduling coordination and documentation workflows
  • Assist with obtaining records, releases of information, and care coordination tasks
  • Maintain accurate client records within the EHR system
  • Support in-office client flow and day-to-day clinic operations
  • Help prepare and organize provider schedules and client communications

Administrative & Team Responsibilities

  • Collaborate closely with front office, psychiatry, and leadership teams
  • Cross-train across operational workflows to provide team coverage when needed
  • Maintain HIPAA compliance and uphold strict confidentiality standards
  • Participate in team meetings, trainings, and workflow improvement initiatives

Qualifications

  • Active CNA or MA certification required
  • Fluency in Spanish and English required
  • Experience in behavioral health, medical offices, social services, pediatrics, or care coordination preferred
  • Strong communication and customer service skills
  • Experience with Electronic Health Records (EHR) preferred
  • Technically capable (comfortable using Google Suite) and quick to learn EHR systems and operational workflows

Pay: $24.00 - $32.00 per hour
