Clinical Care Coordinator

Summary:

Our Clinical Care Coordinator assists with the development of clinical programming, assesses fitness for programming, and is responsible for individualizing treatment based on each client’s needs. This role acts as a primary point of contact for clients enrolled in the treatment program. This position is tasked with the development and oversight of a client’s overall treatment planning, and working with an average caseload of 8-12 clients. The Clinical Care Coordinator will provide case management throughout the course of treatment, which includes the coordination of a client’s treatment during their episode of care and thorough discharge planning at treatment completion. As a primary point of contact, the Clinical Care Coordinator will communicate and interact with families and referring professionals on a regular basis, and recommend additional therapies that are clinically indicated (e.g., family, couples, trauma, 12-Step).

The clinical care coordinator will participate in clinical meetings and case consultations weekly (approximately 8 hours of work week). Participation in weekly supervision with the Program Director, as well as trauma supervision with the manager of the trauma program is also required. Suicide assessments, overall risk evaluation, abuse assessment and reporting are expected to be completed when clinically indicated for all clients admitted to The Collective.

It is expected that the Clinical Care Coordinator will have the skills to de-escalate patients through ad hoc 1:1 interaction, as needed. Clinical Care Coordinator, in collaboration with the Program Director, Clinical Director, and The Collective outreach team will maintain positive relationships with various treatment facilities that may provide referrals. This may require proactive outreach to such facilities in support of the marketing directives set up by the Clinical Director, Medical Director, Program Director and the marketing team.

Education/Experience Required:

  • Minimum of a Master’s degree in Social Work, Marriage and Family Therapy, or closely related field of study from an accredited institution and registered as an associate in the state of CA. Unlicensed candidates must have an adequate credential to legally work as a clinician (ASW, ACSW, AMFT, etc.)
  • Minimum 2 years of experience providing direct clinical care in a mental health treatment setting with job duties involving assessment, case management, discharge planning, and crisis intervention
  • Must have graduated from high school or possess a GED
  • Must be 18 years of age
  • Must have a required health screening
  • Must have a current Tuberculin Screen or obtain upon 7 days of hire
  • Valid CPR/First Aid Certification
  • Must have criminal record clearance
  • Skilled at clinical assessments and making appropriate clinical recommendations
  • Ability to support and collaborate with a multidisciplinary treatment team
  • Proficiency with software and/or equipment, including Microsoft Office applications, electronic medical record software, Google applications, etc.
  • Strong organizational skills with ability to prioritize tasks, work independently, manage multiple tasks, and meet deadlines.
  • Strong written and verbal communication skills

Preferred Qualifications:

  • Demonstrated case management ability with a caseload of 10+ clients
  • Some background in psychopharmacology/med management
  • Prior experience working in a Residential Level of Care, Crisis Stabilization, PHP or IOP with a multidisciplinary team

Essential Duties and Responsibilities:

General Responsibilities Include:

  • Consult with Intake Team as to any questions regarding intakes
  • Collaborate with the Clinical Director to assign their clients’ treatment team members.
  • Identify the need for psychological testing, adjunct services, collateral services, special medical services, etc.
  • Crisis management, including the assessment of suicide risk and abuse. Report to appropriate parties, based on legal and ethical duties.
  • Must be available for ad hoc case consultation, client sessions, and family or referent communication during regular clinic hours
  • After hours clinical on-call support shift one week per month

Case Management Responsibilities Include:

  • Communicate regularly with referents and families to provide updates on client treatment
  • Incorporate information from multiple sources to guide treatment for individual clients
  • Evaluate and advocate for a client’s appropriateness for their level of care
  • Determine need for continuation and work with families/referrer to secure treatment
  • Regularly meet with clients for case management services
  • Provide timely and compliant (per company policies) documentation via EHR of client’s treatment and progress including notes and concurrent reviews for insurance reimbursement
  • Discharge planning

Program Coordination Responsibilities

  • Regularly communicate and collaborate with Clinical Director, Program Director and the treatment staff
  • Coordinate consultation meetings
  • Attend recurring clinical supervision meetings and staff meetings
  • Monitor CM, Therapist, and Group Therapist EHR notes to ensure quality gather pertinent information about a client's treatment progress
  • Maintain Clinical Policies and Procedures
  • Continuous analysis of level of care appropriateness.

Meetings, Marketing, Etc. Responsibilities:

  • Attend weekly treatment planning meetings with Clinical Director, Medical Director, and Program Director
  • Attend and/or present at occasional conferences/marketing events
  • Meet with other mental health facilities

Physical Requirements/Work Environment:

Work is primarily sedentary in nature, occasionally lifting 30 or more pounds and must be able to lift up to 50 pounds (ie. luggage). Must be able to walk, stoop, sit, stand, climb, stretch, reach, and possess finger/dexterity. Must be able to walk and work standing on feet up to 75% of the day, as well as work on a computer for up to 75% of the day. Must have a level of mobility necessary to meet patients’ needs. Quality of hearing (whether natural or with accommodation) must be acceptable due to frequent phone conversations with business representatives, potential clients, and families. They must exhibit the ability to display coping skills to handle multiple calls and duties. The risk of exposure to unpredictable situations (i.e., client behavior) exists.

Pay: $73,000.00 - $78,000.00 per year
