Clinical Care Coordinator - Hospice

About the Role

We are seeking a compassionate and organized Clinical Care Coordinator to support the daily clinical operations of our hospice team. This role bridges clinical leadership and day-to-day patient care coordination, ensuring smooth operations, strong team communication, and high-quality patient care.

The Care Coordinator partners closely with the Director of Nursing, admissions team, and field staff to keep patient services, scheduling, and clinical coordination running efficiently.

This position plays a key role in supporting our hospice staff, coordinating patient services, assisting with admissions flow, and helping maintain compliance with hospice regulations.

Key Responsibilities

• Coordinate and oversee hospice aide scheduling and supervision
• Provide guidance and support to hospice aides and clinical staff
• Coordinate admission process and intake workflow
• Monitor documentation and visit compliance
• Participate in Interdisciplinary Team (IDT) meetings
• Support the Director of Nursing with daily clinical operations
• Assist with staffing coordination and patient care planning
• Conduct required hospice aide supervisory visits
• Help maintain Medicare compliance and documentation standards

Qualifications

• Registered Nurse (RN) license required
• Hospice experience preferred
• Strong organizational and communication skills
• Experience with computer & EMR systems
• Ability to support team members and manage multiple priorities in fast-paced environment
• Passion for hospice philosophy and compassionate care

Pay: From $85,000.00 per year
