Duties
- Conduct assessments to identify veterans’ housing, employment, healthcare, and support needs
- Enroll eligible veterans into the GPD program and maintain accurate participant records
- Develop individualized service plans based on veterans’ goals and needs
- Provide ongoing case management, housing support, and employment assistance
- Connect veterans to healthcare, mental health, substance abuse, financial, and community resources
- Monitor veteran progress and update service plans as needed
- Assist veterans with securing and maintaining stable housing
- Educate veterans on budgeting, tenant responsibilities, and landlord relations
- Support veterans in transitioning to permanent housing
- Coordinate services with VA staff, community partners, and service providers
- Maintain confidential and accurate case files, reports, and program documentation in compliance with HIPAA regulations
- Protect client confidentiality and safeguard sensitive health and personal information
- Track veteran outcomes and program performance metrics
- Participate in team meetings to discuss veteran progress and program needs
- Build positive relationships with veterans, staff, and community partners
- Stay informed on best practices, regulations, and trends in veteran services and case management
- Participate in trainings and professional development opportunities
- Contribute to improving GPD program services and veteran support initiatives
Requirements
- LCSW-C or LCPC required.
- Proven experience in case management, preferably working with veterans or in a related field.
- Strong understanding of veteran issues, housing stability strategies, and supportive
- services.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite and case management software.
- Strong problem-solving and decision-making abilities.
Pay: $55,000.00 - $63,000.00 per year
