The Clinical Coordinator is responsible for handling and overseeing administrative aspects of nursing services provided to the patients. They coordinate the activities and schedules of all nursing visits for patients and Home Health Aides supervision, communicates patient status to nursing staff and DPS. Overall, Clinical Coordinator assists the clinical management team in ensuring the safe, effective and productive treatment of patients.
- Enter Interim orders, if applicable. Enter Medication list for each order and Certification period.
- Enter complete patient’s demographic info into patient’s profile including emergency contact, primary caregiver, PCP and pharmacy info.
- Follow up with PCP offices and ensure MD orders are signed and returned to agency.
- Enter, update and upload scanned Plan of Care and signed MD Orders info into HHA eXchange.
- Update Spreadsheet/Log with SOC date, Revisit Dates, MD Orders faxed/received, next visit due date. Mail or Fax MD orders to PCP.
- Prepare RN visit list for upcoming month; Assist RNs with scheduling visits; follow up on RN visit completion and update spreadsheet/log
- Prepare List of RN MLTC UAS-NY assessments and schedule RNs.
- Enter authorizations for UAS-NY assessments.
- Review daily census reports, identify issues for immediate RN supervision/follow-up and schedule RN visits accordingly.
- Work on conflict report; address and follow up on all issues
- Ensure DPS assessments are reviewed and scanned into patient’s charts
- Ensure all initial RN visits are completed within 72 hours of start of care date, or within 5 days of service if patient only receives 2 or 3 days of service weekly or only has weekend service.
- Responsible for answering calls and collecting all pertinent medical information
- Maintains patient confidentiality and all HIPAA privacy requirements
- Performs related work or other duties as required

Pay: $25.00 - $31.00 per hour
