Clinical Coordinator, LPN-Home Health

The Clinical Coordinator (LPN) bridges the gap between administrative intake and clinical excellence. You will provide essential support to the Clinical Director, ensure the accuracy of physician orders, manage visit frequencies, and maintain high standards through Quality Assurance (QA) oversight. Key Responsibilities

• Clinical Administration: Direct support to the Clinical Director; enter/track physician orders in the EMR and process daily on-call reports for Case Managers.
• Quality Assurance (QA) & Compliance: Audit clinical charts for Medicare COP/state compliance and monitor visit frequencies to ensure strict adherence to the Plan of Care (POC).
• Intake & Scheduling Support: Serve as primary backup for Intake and Scheduling; coordinate clinician territories and assist with referral processing/insurance verification.
• Communication Hub: Triage incoming clinical calls (within LPN scope) and serve as a central point of contact for clinical documentation tracking.

Qualifications

• Licensure: Active MI LPN license in good standing.
• Experience: Minimum 1 year in home health or a related clinical role preferred.
• Technical Skills: Proficiency in EMR systems and strong computer literacy.
• Soft Skills: Sharp organizational skills, a collaborative spirit, and a deep understanding of home health regulations.

Pay: $30.00 - $33.00 per hour
