Clinical Coordinator RN

Responsibilities:

The Clinical Coordinator RN is that person who is responsible for the coordination of care of the patients and families in the hospice program of care.

. RN coordination and follow through with referrals to the hospice

. RN coordination of admission of patients to the hospice program

. RN completion of the initial assessment/Plan of Care during periods of necessity

. RN communication between each member of the IDG and primary physician

. Assignment of nurse case managers to develop and implement the Plan of Care and oversee the delivery of care to the patient/family

. Policies and procedures concerning patient care are followed, documented, and reviewed

. Supervision, evaluation, and scheduling of patient care employees, volunteers, and contracted employees

. Safe transfer of hospice patients as they change levels of care

. The supervision of patients while they are on inpatient level of care

. Participation in program development as needed

The Clinical Coordinator is a Registered Nurse from an accredited program currently licensed and in good standing with the state of the program’s licensure, least two years of supervisory experience in community health programs is preferred. Must have strong clinical skills with hospice and/or home health experience.

Overseeing the day today clinical operations of the hospice program to ensure high quality compassionate care for patients and families.

Providing leadership, support and direction to the clinical team, while ensuring compliance with regulatory standards and organizational policies.

Monday - Friday 8:00am to 5:00pm and On Call

• Coordinate daily clinical operations
• Supervise and support clinical staff
• Monitor patient caseloads
• Facilitate IDG meetings
• Participate in admissions DC's
