Position Overview

We are seeking a Clinical Coordinator (RN) to oversee and coordinate patient care from admission through discharge. This role ensures timely starts of care, seamless communication across the care team, and compliance with all regulatory standards.

This is a fast-paced, highly collaborative role ideal for someone who thrives on organization, accountability, and improving patient outcomes.

Key Responsibilities

  • Coordinate patient care services from referral through discharge
  • Ensure timely Start of Care (SOC) of referral
  • Monitor hospital admissions, transfers, and discharges daily
  • Review incoming referrals and assign to appropriate clinical staff
  • Ensure field clinicians complete visits as scheduled
  • Collaborate with physicians, referral sources, and interdisciplinary team members
  • Develop, review, and update patient care plans based on ongoing needs
  • Lead and participate in case conferences and care coordination meetings
  • Ensure timely ordering of labs, supplies, and medical equipment
  • Support staffing coordination based on geographic efficiency and patient needs
  • Maintain accurate and compliant documentation per agency policies
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
  • Ensure compliance with Medicare Conditions of Participation, AHCA, ACHC, and HIPAA regulations
  • Address patient, physician, and staff concerns in collaboration with leadership

Qualifications

  • Active Registered Nurse (RN) license in the state of Florida (required)
  • Minimum 1 year of home health experience preferred
  • Strong knowledge of Medicare guidelines, Conditions of Participation, and regulatory standards
  • Strong clinical judgment, organizational, and problem-solving skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Excellent communication and interpersonal skills
  • Proficiency with electronic medical records (EMR) systems
