Job Description:

The Clinical Data Abstractor II will be responsible for reviewing, abstracting, and analyzing clinical data from various sources, ensuring accuracy and completeness in accordance with regulatory standards and organizational protocols. Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks.

Key Responsibilities and Tasks:

  • Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures.

  • Extract and input clinical data from electronic health records, patient charts, and other relevant sources into quality reporting and analytics platform for measure compliance.

  • Conduct supplemental data reviews according to quality program priorities.

  • Ensure data accuracy, integrity, and compliance with established guidelines and standards.

  • Abstract clinical information from paper/electronic medical records and input data accurately into quality reporting and analytics platforms to improve measure performance.

  • Support chart collection efforts by sending faxes and calling provider offices/facilities for information.

  • Obtain medication lists from provider offices for pharmacist review.

  • Assist with scheduling pharmacist consultations.

  • Pull and prepare actionable reports to share with population health team, regional services representative, and provider offices.

  • Assist in preparing reports and presentations based on abstracted data for stakeholders.

  • Directly outreach to patients to assist in closing gaps in care.

Skills and Experience Required:

  • Bachelor’s degree or equivalent experience

  • Expert knowledge of quality measures (e.g., HEDIS, PQA, IHA)

  • 3+ years of experience with quality reporting and analytics platforms (COZEVA preferred)

  • 3+ years of experience in healthcare industry

  • 3+ years of clinical chart review/data abstraction, clinical research, data management, or health informatics experience

  • Strong electronic medical record knowledge and ability to quickly learn new software

  • Ability to organize tasks and complete projects in a timely manner

  • Excellent interpersonal, verbal and written communication skills

  • Attention to detail and ability to follow precise instructions

Additional Information:

Salary: $26 - $29 Hourly
