DUTIES AND RESPONSIBILITIES:

· The Residential Supervisor will oversee the overall operations of a case load of up to two (2) group homes.

· Builds a positive and realistic image of the organization and programs within the community

· Assures that the homes meet state licensing and other regulatory requirements.

· Conducts assessments, assures the development and implementation of all service plans and other participant documentation.

· Direct contact with individuals, DSP’s, family members and members of other agencies providing services to individuals such as Day Program staff, Case Managers, Physicians, and other representatives to ensure satisfaction with agency services.

· Assures the maintenance of medical records and coordinate with immediate supervisor Program Director/ RN for any needs for the individuals.

· Participate in the coordination of the admissions and discharge of the participants.

· Assist in conducting and/or review performance evaluations of staff in accordance with policy.

· Supervise each Group home and conduct unannounced visits to each location to ensure that the individuals are receiving excellent care and ensure that all medication and other clinical records are properly maintained reporting any discrepancy on service to the immediate Supervisor.

· Responsible for the supervision and well maintain of equipment at residential sites such as vehicles, adaptive equipment, electronics, business communications equipment, appliances, recreation equipment and associated furnishings

· Monitor special medical conditions, i.e. seizures, diabetes, special diets, physical disabilities, and duals diagnosis.

· Responsible for the collection and dispersement of funds for each individual included monthly reports maintaining accounting records and disperse copies of receipts to family members and authorize representative.

· Ensures the wellbeing of all individuals, healthy meals, individuals personal hygiene are maintaining in high standards providing proper supervision to DSP’s work performance

· Report any difficulties problems to the designated Supervisor

· Safeguard the health, safety and civil and personal rights of all individuals and their families

· Conduct random Fire or other safety drills in the homes

· Dispense medications

· Provide training and support to DSP’s

· Complete incident report as needed.

· Responsible for developing and ensuring the implementation of the Individual Service Plans/Person-Centered Plans.

· Responsible for completing quarterly and annual reviews in a timely manner

· Responsible for completing individual record reviews on a monthly basis

· Responsible for reviewing support logs, training schedules and other documentation to ensure coordination with the individual’s ISP/PCP. .

· Develop schedules and review staff time sheets to ensure accuracy and proper coverage of the homes.

KNOWLEDGE, SKILLS, AND ABILITIES:

· Ability to perform day to day work independently with regular consultation and frequent follow up from the Program Director.

· Ability to work in conjunction with the Program Director/ RN and Agency CEO to ensure the proper function of the programs.

· Ability to oversee, train, direct, supervise, and manage the activities of Direct Support Professionals (DSP’s).

· Ability to communicate effectively both verbally and in writing.

· Possess strong organizational and time management skills.

· Possess a broad knowledge of intellectual, developmental, mental, physical and other disabilities.

· Knowledge of person-centered planning principles.

· Knowledge of principles and practices of Positive Behavior Support.

· Ability to complete special projects and other duties as assigned.

· Possess caring and positive attitude; enjoys working with all people.

· Possess strong supervisory skills.

· Ability to establish and maintain effective working relationships with individuals, community and staff.

· Possess investigative and interviewing skills and the ability to analyze findings

· Identify problems and formulate solutions.

· Knowledge of mental health, intellectual and developmental disabilities and related physical, health and behavioral concerns.

· Experience working with individuals with disabilities.

· Knowledge of and experience completing comprehensive assessment tools.

· Knowledge of MR/ID Waiver regulations.

· Available to respond emergency calls during all hours of operation.

· Responsible to complete incident reports and immediately contact supervisor within 3 hours of incident occurrence.

· Possess a valid driver’s License, Transport individuals as necessary/ Agency will honor mileage records

(Masters in social work required)

Job Type: Full-time

Pay: $65,000.00 - $80,000.00 per year
