EDUCATIONAL BACKGROUND & TRAINING REQUIRED:
Any healthcare degree. MBBS preferred.
KNOWLEDGE & EXPERIENCE REQUIRED:
Fresh to 1 year of experience in healthcare with a focus on patient education and chronic disease management.
GENERAL JOB DESCRIPTION:
The Clinical Facilitator is responsible for contacting patients within 2 days of hospital discharge to ensure a smooth transition of care. This role includes conducting medication reconciliation, educating patients on the importance of medication adherence, addressing barriers to care, and writing care summaries based on patient progress notes in the Electronic Medical Record (EMR). The Clinical Facilitator works closely with the care team to support patients in managing their health, promoting recovery, and preventing readmissions.
KEY RESPONSIBILITIES:
· TCM enrollment and Post-Discharge Support:
Contact patients within 2 days of discharge to assess their health status
· Medication Reconciliation & Adherence:
Reconcile medications with patients, ensuring accuracy between hospital discharge instructions and current medications. Educate patients on the importance of adhering to prescribed medications, addressing barriers such as cost or side effects, and providing ongoing support.
· Care Summary & Documentation:
Write detailed care summaries based on progress notes in the EMR, ensuring care managers have the most up-to-date information for effective patient communication and continuity of care.
· Care Coordination & Follow-Up:
Schedule appointments, connect patients with their providers, and ensure that any identified barriers to care are addressed.
· Patient Education & Monitoring:
Educate patients on medication adherence. Track patient progress and provide support as needed
SKILL SET & ABILITIES:
· Strong communication skills (US English), both verbal and written with experience in patient education
· Ability to educate patients and caregivers in a clear, empathetic manner
· Proficiency with Electronic Health Records (EHR) and healthcare software
· Strong organizational skills and attention to detail
· Ability to manage multiple priorities and work independently
· Knowledge of chronic disease management, medication management and common adherence barriers
