Function(s):
Provide clinical assistance to all programs within the HCBS division by completing assessments, developing care treatment and service plans and providing waiver service coordination and/or care coordination services for members within the HCBS division.
JOB RESPONSIBILITIES:
Primary Duties:
1. Complete care coordination and waiver service coordination duties across all programs, including temporarily managing a full or partial caseload of members enrolled within any HCBS program as determined by the Clinical Support Supervisor.
2. Clinical Float II may oversee Clinical Float 1 work as needed.
3. Conduct in-person assessments within required timeframes for program applicants and enrolled members within the Home and Community Based Services Division.
4. Evaluate assessment and care coordination data to determine program eligibility, care needs, and service options.
5. Complete significant change event assessments with members within required timeframes.
6. Schedule and participate in Interdisciplinary Care Team (ICT) meetings and complete assessments as required.
7. Complete home visits to ensure health, safety, service satisfaction and provide education to members, based on program specific guidelines.
8. Educate members on waiver services and available providers, including self-direction and transportation needs.
9. Educate individuals and caregivers on health promotion, illness prevention, community resources and navigating the healthcare system.
10. Develop and document the Member's disaster preparedness/backup plan and contact information in the Waiver Service Plan.
11. Complete all actions and follow up related to critical and reportable incidents as defined in regulatory requirements.
12. Develop a health and safety action plan (HSAP) and monitor as applicable.
13. Assist with telephonic coverage during the day when needed.
14. Develop and update person-centered care plans within required time frame.
15. Advocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings.
16. Disenroll ineligible members from program, as appropriate.
17. Apply judgments and perform clinical practice based upon education, experience and professional practice standards.
18. Complete all required documentation in accordance with the applicable HCBS program.
19. Monitor service quality and program eligibility.
Required
Preferred:
· Interest in Care Coordination and working with elderly and disabled populations
Knowledge of chronic disease management.
Pre-employment drug & nicotine testing required
· Annual influenza vaccination is required
