Position Summary:

The Case Manager provides housing-focused case management to individuals experiencing homelessness or at risk of homelessness. This role conducts housing intakes and assessments, develops participant-centered service plans, and supports participants to secure and maintain safe, stable housing. The Case Manager coordinates with landlords, housing providers, and community partners, and collaborates closely with Rainbow Health clinicians to support whole-person stability.

Responsibilities:

  • Conduct housing-focused intakes, screenings, assessments, and participant-centered service planning
  • Support participants to secure and maintain safe, stable housing (housing search, applications, unit viewings, move-in coordination, and stabilization)
  • Coordinate and navigate housing resources, rental assistance, CES/housing pathways (as applicable), and public benefits
  • Provide tenancy sustaining support, including landlord mediation, lease education, household setup, budgeting, and life-skills coaching
  • Coordinate care with behavioral health clinicians, medical providers, and community partners to reduce barriers to stability
  • Provide field-based services, including home visits, hospital/shelter/community outreach, and accompaniment to appointments as needed.
  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR) and any required tracking systems
  • Participate in case conferences, supervision, team meetings, and continuous quality improvement activities
  • Maintain confidentiality and professional boundaries consistent with HIPAA and agency policies

Requirements:

Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or a related field (required), Masters Degree strongly preferred.

At least two (2) years of experience in housing case management, homelessness services, outreach, or a closely related human services role (required)

Knowledge of Hawaii housing systems and community resources (shelters, housing navigation, benefits access, and supportive services)

Experience working with individuals experiencing homelessness and/or complex needs (SUD, SMI, chronic health conditions).

Strong organizational skills with the ability to manage a flexible caseload and prioritize urgent needs

Ability to work independently in community settings and as part of an interdisciplinary team

Proficiency with documentation and basic computer systems; ability to learn and use an EHR

Reliable transportation, valid driver’s license, and willingness to travel within assigned service areas on Oahu.

Work Environment & Physical Requirements:

This is a community-based role that includes travel to shelters, hospitals, partner sites, encampments, and participant residences. Must be able to sit/stand for extended periods, transport supplies/documents, and work in variable field conditions.

Job Type: Full-time

Pay: $50,000.00 - $58,000.00 per year
