Clinical Intake Coordinator

Job Overview
We are seeking a Clinical Intake Coordinator to join our team. The ideal candidate will have experience in an outpatient setting, providing mental health case management, skills, psychosocial rehab, and crisis intervention.

Responsibilities
- Complete assessments and provide appropriate levels of care

- Work collaboratively with individuals and families to prepare and maintain safety and recovery plans

- Offer crisis intervention and support to individuals in need

- Provide case management services to link individuals to needed services

- Provide skills training in the office face to face or via telehealth

- Collaborate with multidisciplinary teams for comprehensive care

- Works collaboratively with law enforcement, courts, and crisis teams to prepare individuals in treatment to meet their recovery goals

- Document within 24 hours into an electronic health record

Requirements
- Master's degree in psychology, social work, or a related human services field

- LPC, LPC-A or LMSW

- Previous experience in an outpatient mental health setting

_ Previous experience with the homeless population

- Strong knowledge of community resources

- Excellent engagement skills

- Most have reliable transportation.

- Bilingual preferred but not required

Pay: From $55,000.00 per year
