Clinical Intake Coordinator

What You’ll Do

As a Clinical Assessor in PAC, you will play a key role in creating an environment where patients and families feel supported, safe, and inspired to grow. You will:

·Clinical Assessment & Triage:

- Conduct biopsychosocial assessments to determine the appropriate level of care (Inpatient, PHP, IOP, Outpatient).

- Identify emergent needs and prioritize assessments accordingly.

- Recognize the need for additional screenings (substance use, nutrition, abuse, developmental).

- Make appropriate referrals for clients not meeting admission criteria.

Access Coordination:

- Collaborate with Patient Access Coordinators to schedule and facilitate same-day assessments.

- Assist in collecting and verifying documentation (consents, insurance info, IDs).

- Provide outbound referrals when needed for alternate services or providers.

Clinical Documentation & Authorization:

- Complete and maintain accurate assessment records and admission documentation.

- Support utilization review efforts and assist with initial authorization from payors.

- Ensure clear documentation of client needs and assessment outcomes.

Communication & Collaboration:

- Maintain thorough, professional communication with the outreach, admissions, and clinical teams.

- Serve as a clinical point of contact for internal staff and referral sources.

- Participate in case management and family engagement as needed during the intake process.

Compliance & Quality:

- Uphold HIPAA and all regulatory requirements in documentation and interactions.

- Adhere to safety protocols and participate in quality improvement initiatives.

- Report incidents, near misses, or safety concerns per company policy.

What We’re Looking For

  • Master’s Degree in Counseling, Social Work, or a related behavioral health field.
  • LMSW, LCSW, LPC-A, or LPC (Texas)
  • Experience in behavioral health levels of care (Inpatient, PHP, IOP, OP) required. 2+ years in a mental health intake, assessment, or admissions role preferred.

Pay: $65,000.00 - $70,000.00 per year
