Job Summary
We are seeking a proactive and compassionate Clinical Intake Coordinator to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive patient intake assessments across inpatient, outpatient, emergency, and post-acute care settings, utilizing medical terminology and physiology knowledge to gather detailed clinical information.
- Coordinate with hospital staff, social workers, behavioral health specialists, and case managers to ensure timely access to appropriate services.
- Manage patient records in compliance with HIPAA regulations, ensuring confidentiality and accuracy throughout the intake process.
- Facilitate discharge planning by collaborating with multidisciplinary teams to develop effective post-acute care plans.
- Assist with diagnostic evaluations and utilization management to optimize patient treatment pathways and resource allocation.
- Support case management activities by evaluating patient needs in trauma centers, ICU units, PICUs (Pediatric Intensive Care Units), and other specialized departments such as addiction counseling or mental health clinics.
- Maintain detailed documentation of patient assessments, medical records, and intake procedures while adhering to hospital policies and procedures.
- Provide excellent patient service by guiding individuals through the intake process with professionalism and empathy while addressing their questions or concerns. Experience
- Prior hospital experience is required, preferably within Level I or Level II trauma centers or acute care environments.
- Experience working in ICU settings or with inpatient and outpatient populations is highly desirable.
- Knowledge of medical terminology, physiology, and diagnostic evaluation techniques is essential for success in this role.
- Demonstrated experience in behavioral health, mental health counseling, social work, or addiction counseling enhances your ability to support diverse patient needs.
- Familiarity with discharge planning, utilization management, case management processes, and managed care principles is preferred.
- Previous involvement in emergency medicine or hospital medicine settings will be considered an asset.
