Clinical Intake Coordinator

Overview
Part Time Patient Intake Coordinator for Hospice & Palliative Care after hours. (LVN required)

20 hours per week, work from home position. Current LVN license.

Join our dynamic healthcare team at Essence Hospice as an after-hours Intake Coordinator, where your expertise will be pivotal in ensuring seamless provision of patient care/intake during evenings and/or weekends. In this vital role, you will serve as the first point of contact for our answering service and check incoming fax communications for new patient referrals to hospice and/or palliative care. Your compassionate approach and attention to detail will help facilitate efficient patient flow, accurate information gathering, and effective case management. This position offers an exciting opportunity to contribute to quality patient care coordination with our hospice organization. Must be familiar with patient referral process, insurance eligibility, insurance authorization requirements and be efficient at collecting all required consents and paperwork required. Must be proficient on computer and experience with HospiceMD software a plus.

Responsibilities

  • After-hours administrative LVN duties include checking for incoming new patient referrals. Must quickly identify the patient information, insurance coverage, clinical history for review. Coordinate with multidisciplinary teams to facilitate appropriate admissions.
  • Proficient at communicating to patient and family/caregivers regarding explanation of benefits. Efficient at processing all required forms/documents including physician orders, patient consents to elect services along with all required paperwork needed for patient care/services.
  • Completes referral information that includes intake data, essential background information in EMR HospiceMD, creates chart. Manage medical records and documentation in compliance with HIPAA regulations, ensuring accuracy and confidentiality at all times.
  • Serve as administrative point of contact after hours for answering service, marketing team and after hours clinical LVN's/RN's performing field work.
  • Maintain detailed logs of any/all patient activity, including intake activities, patient assessments, and case progress to support quality assurance and continuous improvement initiatives.

Qualifications

  • Proven administrative experience coordinating patient care services. LVN license required and hospice experience preferred.
  • Strong knowledge of medical terminology, physiology, and evaluation procedures.
  • Experience working within managed care systems or utilizing utilization management protocols. Knowledge of running insurance benefits, checking for any past hospice benefit periods or open episodes.
  • Excellent patient service skills with the ability to effectively coordinate members of the hospice/palliative care team efficiently.
  • Familiarity with electronic medical records systems and medical records management. Hospice/PalliativeMD experience a plus.
  • Ability to handle sensitive information in accordance with HIPAA standards while maintaining a compassionate approach to patient interactions. This role is ideal for motivated professionals eager to make a meaningful impact on patient outcomes through proactive intake coordination.

Pay: $21.00 - $25.00 per hour
