Nurses Care Inc. is seeking an experienced Intake Coordinator to join one of the pioneers in the Home Healthcare industry. We are looking for a professional who is dependable, compassionate, organized, and excited to grow with a company that values its team.

Position Overview

As an Intake Coordinator, you will play a critical role in helping patients begin services smoothly and efficiently. You will communicate directly with hospitals, physicians, case managers, referral sources, patients, and clinical staff to coordinate admissions and ensure a seamless intake process.

Responsibilities Include:

  • Managing patient referrals and admissions
  • Coordinating with hospitals, physicians, case managers, and referral partners
  • Verifying all patient documentation for approval.
  • Working closely with insurance, authorizations, and clinical teams
  • Scheduling patients with our clinical teams. Making sure all openings are within 48 hours, and patients are assigned to a clinical team member.
  • Maintaining accurate EMR documentation
  • Providing excellent communication and customer service
  • Assisting in creating a positive patient experience from the very first interaction

Qualifications

  • Must be one of the following:
  • LPN
  • RN
  • Medical Assistant
  • Previous intake, admissions, scheduling, or home healthcare experience preferred
  • Strong multitasking and organizational skills
  • Ability to work efficiently in a fast-paced environment
  • Professional communication skills
  • EMR/healthcare software experience preferred
  • Self-motivated with a positive attitude and strong work ethic

Compensation:

Pay range starting from: MA: $25, LPN $30, RN $36 per hour

  • Experience
