Clinical Liaison & Intake Specialist

We are hiring a hospice focused and experienced, Clinical Liaison/Intake Specialist. In this role, you will bridge the gap between initial referral and the start of clinical care. This is a blend between administrative coordination and clinical triage.

Job Description:

  • Maintain clear and consistent communication with clinical staff, referral sources, and families throughout the intake process
  • Work with Board and Care groups, hospitals or skilled nursing facilities to facilitate the discharge of potential hospice patients. Act as a bridge between the facility and the hospice agency, ensuring a smooth transition for the patient.
  • Screen inquiries for eligibility, assessing initial patient and family needs over the phone, and ordering initial medical supplies or medications needed for the start of care.
  • Experience with CHAP Hospice accreditation standards.
  • Accurately document all referral and admission activity in the EMR system and maintain tracking logs or spreadsheets
  • Manage the full lifecycle of a referral while also assigning field staff for the initial admission visits.
  • Gather demographics, explain hospice benefits to families, and prepare the necessary paperwork for the admitting nurse
  • Ensure all admission paperwork is received, reviewed, and accurately entered into HospiceMD EMR system.
  • Ensure maximum third-party reimbursement through coordination with physicians, payers, and service providers
  • General office duties as directed.
  • Occasional field visits as need by clinical teams.

Qualifications

Demonstrated compassion and commitment to hospice patient care

· Active LVN license in the State of California, in good standing

· Maintain valid California driver’s license and reliable transportation

· Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care

· Prior experience in nursing or related medical fields

  • Bilingual (English and Spanish) preferred but not required

Pay: $80,000.00 - $85,000.00 per year
