Santa Clarita Dialysis LLC, a rapidly expanding dialysis provider in Santa Clarita, is seeking applicants for the position of Clinical Manager. The organization is prepared to offer management training to qualified dialysis nurses.

The role of Clinical Manager is integral to fulfilling our mission to deliver exceptional dialysis treatment experiences. This position will collaborate with administrative staff, nursing professionals, certified hemodialysis technicians, physicians, the governing body, as well as patients and their families, to enhance patient quality of life. Furthermore, we place great emphasis on patient education and customer service, expecting all team members to consistently provide compassionate, individualized, and high-quality care to every patient.
Responsibilities:
Customer Service Oversight
Clinical Oversight
· Regulatory Oversight ensures each center maintains compliance with all State and Federal regulations, CMS Conditions for Coverage for End-Stage Renal Disease Facilities, and DHS Interpretive Guidelines.
· Fosters a culture focused on quality work, patient safety, and adherence to clinical discipline.
· Verifies that facility operations align with DHS, CMS, and facility-specific policies and procedures.
· Ensures strict adherence to physician orders, including medication prescriptions, dialysis machine setup, and related clinical directives.
· Oversee the accurate entry of patient treatment data into the EMR system on a daily basis.
· Enforces rigorous infection control protocols.
· Organizes and participates in mock surveys to maintain a safe environment for all patients.
· Collaborates directly with the clinical team to analyze clinical trends and implement actionable plans to drive ongoing quality improvement.
· Monitors adequacy of dialysis treatments by reviewing Kt/V, URR, and other performance metrics to ensure optimal patient outcomes.
· Reviews vascular access management practices, including monitoring AV fistula and graft sites, ensuring timely interventions and minimizing complications.
· Audits water treatment systems and dialysis equipment maintenance logs to confirm compliance with safety standards and manufacturer recommendations.
· Provides ongoing education and competency assessments for dialysis staff regarding updated clinical protocols and evidence-based best practices.
· Conducts thorough root cause analyses following adverse events or incidents to identify opportunities for enhanced patient safety within dialysis care.
· Coordinates interdisciplinary meetings to discuss challenging patient cases, address barriers to effective dialysis, and facilitate holistic care planning.
Administration Oversight
· Ensure that staff meetings, in-service education sessions, and team patient care planning meetings are conducted on a monthly basis.
· Attend monthly Governing Board meetings and prepare comprehensive reports as required.
· Participate in monthly QAPI meetings, ensuring timely preparation of relevant documentation.
· Maintain effective collaborative relationships with the Medical Director and physicians.
· Ensure centers consistently comply with the submission of accurate data and other required information to governing agencies (e.g., ESRD Networks) in a timely manner.
· Coordinate with staff to uphold OSHA compliance, facilitate occurrence reporting, and maintain current emergency preparedness protocols.
· Collaborate with the Bio-Med Tech to sustain a safe and sanitary facility environment, ensuring all equipment meets maintenance and operational standards.
· Conduct regular facility audits to verify clinical and quality compliance.
· Manage staff and patient scheduling.
· Monitor and assess employee performance on an ongoing basis.
· Address patient grievances by working closely with individuals to achieve satisfactory resolution.
· Provide expertise and support to address both patient-related and administrative concerns.
