Clinical Manager - Registered Nurse

We are seeking a qualified Registered Nurse (RN) to provide comprehensive patient care in a home health setting.

• Conduct patient assessments, monitor vital signs, and evaluate health histories.
• Administer medications and treatments safely as prescribed.
• Deliver specialized care for internal medicine, dementia, and dialysis patients.
• Perform nursing procedures such as catheterization and airway management.
• Collaborate with the healthcare team to develop and document personalized care plans, including MDS.
• Educate patients and families on health coaching strategies to improve outcomes.
• Assist in the operating room when needed, maintaining sterile conditions.
• Maintain accurate records in compliance with healthcare regulations.
  
  

Qualifications

• Current RN license in good standing.
• Home health experience required; dementia care or dialysis experience preferred.
• Proficiency in vital signs assessment and basic nursing procedures.
• Strong communication skills and ability to work within a team.
• Experience in clinical or OR settings is a plus.
  
  

Details: Full-time or part-time role with flexible schedule, paying $40-$50/hour. Preference for candidates in Fort Wayne, IN, with at least 1 year of dementia and home health experience. In-person work environment.
