We are seeking an experienced Registered Nurse Clinical Manager with strong leadership skills and prior home health experience to join our growing team. This role requires a regular in-office presence and includes a clinical leadership role with a relationship-based marketing component, supporting referral development and community partnerships. The position offers a collaborative, supportive environment with a strong focus on quality, compliance, and work-life balance.
Responsibilities
- Oversee daily clinical operations to ensure efficient, compliant delivery of home health services
- Provide clinical leadership and supervision to nursing, therapy, and administrative staff
- Ensure accurate, timely completion and review of OASIS assessments, plans of care, and clinical documentation
- Drive performance related to Home Health Value-Based Purchasing (HHVBP), quality measures, and patient outcomes
- Support Pre-Claim Review (PCR) processes, including chart readiness, submission accuracy, tracking, and follow-up
- Utilize Homecare Homebase (HCHB) to monitor documentation quality, workflows, productivity, and compliance
- Develop, implement, and enforce clinical policies and procedures supporting patient safety and regulatory compliance
- Monitor quality indicators, audit findings, and outcomes data; implement corrective action plans as needed
- Maintain continuous survey readiness and ensure compliance with Medicare Conditions of Participation, state regulations, and payer requirements
- Collaborate with physicians and interdisciplinary team members to optimize patient care and satisfaction
- Support operational and financial performance through quality-driven clinical management
- Participate in referral development activities, including provider relationship support, community outreach, and collaboration with marketing leadership to support census growth
Requirements
- Active Registered Nurse (RN) license – required
- Bachelor’s degree in Nursing or related healthcare field – preferred
- Proven clinical management or leadership experience in home health or hospice
- Strong working knowledge of OASIS, PDGM, HHVBP, and Medicare home health regulations
- Familiarity with Pre-Claim Review (PCR) processes and compliance requirements, with the ability to learn and grow in this area on the job
- Proficiency with Homecare Homebase (HCHB) – strongly preferred
- Demonstrated ability to lead clinical teams and improve quality, compliance, and patient outcomes
- Strong interpersonal and communication skills, including the ability to engage effectively with referral sources and community partners
- Excellent organizational and problem-solving skills
- Ability to manage multiple priorities and meet deadlines in a fast-paced environment
- Ability to maintain a regular in-office presence to support clinical operations, collaboration, and referral engagement
