Position Summary

We are seeking an experienced and driven Clinical Manager / Director of Nursing (DON) to lead and oversee clinical operations within our home health division. This role is responsible for ensuring high-quality patient care, regulatory compliance, staff development, and overall clinical performance.

Key Responsibilities
Oversee daily clinical operations and ensure delivery of high-quality patient care
Supervise and support field clinicians (RNs, LPNs, PT/OT/ST, aides)
Ensure compliance with Medicare Conditions of Participation, ACHC standards, and company policies
Review and approve clinical documentation, including OASIS assessments
Monitor patient outcomes, hospitalizations, and quality measures
Provide clinical guidance and support to staff in real-time situations
Participate in hiring, onboarding, training, and performance evaluations
Collaborate with intake, scheduling, and administrative teams for continuity of care
Lead case conferences and ensure proper care planning
Maintain survey readiness at all times
Qualifications
Active Registered Nurse (RN) license in the State of Florida (required)
Minimum 2–3 years of home health experience (required)
Prior leadership or supervisory experience in home health (preferred)
Strong knowledge of OASIS, Medicare guidelines, and compliance standards
Experience with EMR systems (WellSky/Kinnser preferred)
Excellent leadership, communication, and organizational skills
