We are seeking a dedicated, organized, and compassionate Clinical Operations & Scheduling Coordinator to join our team. Position Summary

The Clinical Operations & Scheduling Coordinator serves as a vital member of our clinical leadership and operations team. This position is responsible for coordinating patient scheduling, supporting clinical staff, assisting with quality assurance initiatives, maintaining documentation standards, and helping ensure smooth day-to-day agency operations.

The ideal candidate is highly organized, detail-oriented, proactive, and committed to excellence. They possess strong communication skills, enjoy problem-solving, and understand the importance of balancing operational efficiency with compassionate patient care.

Essential ResponsibilitiesClinical Operations

  • Support daily clinical operations to ensure efficient and effective patient care delivery.
  • Coordinate communication between office staff, clinicians, physicians, patients, and families.
  • Assist with patient admissions, scheduling, care coordination, and service continuity.
  • Monitor clinician productivity and visit utilization.
  • Help maintain compliance with agency policies and regulatory requirements.

Scheduling & Care Coordination

  • Coordinate and manage patient schedules for nursing, therapy, aide, and social work services.
  • Ensure timely staffing and coverage of patient visits.
  • Adjust schedules based on patient needs, clinician availability, and urgent situations.
  • Communicate schedule updates and changes effectively with clinical staff.
  • Assist with after-hours and emergency coverage planning as needed.

Quality Assurance & Documentation Support

  • Review clinical documentation for completeness, accuracy, and timeliness.
  • Assist with OASIS coordination and documentation quality initiatives.
  • Monitor documentation compliance and follow up with clinicians regarding deficiencies.
  • Support quality improvement projects and performance initiatives.
  • Assist leadership in preparing for audits, surveys, and accreditation reviews.

Team Support & Leadership

  • Serve as a resource for clinicians regarding scheduling, documentation, and operational processes.
  • Foster positive relationships among staff while promoting teamwork and accountability.
  • Assist with onboarding and training initiatives for new employees.
  • Support a culture of continuous learning, excellence, and patient-centered care.

Preferred Qualifications

  • Licensed Practical Nurse (LPN)
  • Registered Nurse (RN)
  • Certified Medical Assistant (CMA)
  • Medical Assistant
  • Healthcare administration experience
  • Home health, hospice, physician office, hospital, or healthcare operations experience
