Job Title

Clinical Pharmacist (CP)

Job Summary

Responsible for ensuring safe, effective, and rational medication use as per NABH 6th Edition standards including prescription review, medication reconciliation, ADR reporting, AMS support, and patient counselling.

Qualification: Pharm.D

Experience: 1–3+ years in hospital pharmacy/clinical pharmacy (preferred)

Additional Training: NABH Standards, Pharmacovigilance, AMS/AMR (advantage)

Roles and Responsibility :

  • Conduct prescription review for dose, route, frequency, interactions, duplication
  • Verify allergies and contraindications
  • Prevent and intervene in medication errors
  • Ensure accurate dispensing & labeling
  • Check expiry, storage, cold-chain compliance
  • Monitor LASA, narcotics, high-alert meds
  • Conduct reconciliation at admission, transfer, discharge
  • Document discrepancies and interventions
  • Participate in clinical rounds
  • Provide pharmacotherapy recommendations
  • Monitor high-risk medicines and special populations
  • Review antibiotic prescriptions
  • Ensure adherence to antibiotic policy
  • Monitor restricted antibiotics
  • Identify, manage and document ADRs
  • Report to Pharmacovigilance Committee &PvPI
  • Maintain ADR register
  • Report, document, and assist in RCA of medication errors
  • Implement CAPA
  • Promote no-blame reporting culture
  • Provide counseling on medication use, dosage, effects, storage
  • Ensure discharge counseling is documented
  • Participate in formulary updates, LASA list, high-alert list, antibiotic policy
  • Support audits, quality indicators and NABH compliance
  • Attend committees (DTC, AMS, Pharmacovigilance)
  • Ensure complete documentation in HIS/EMR
  • Maintain records as per NABH documentation requirements

Skills & Competencies

-Clinical pharmacology, prescription review, drug interaction knowledge

-Strong understanding of MOM standards & patient safety protocols

-Identifying medication errors, RCA, CAPA implementation

-Patient counselling, coordination with doctors & nurses

-Proficient in HIS/EMR systems

-Attention to detail, teamwork, ethical practice
