Clinical Pharmacist

• Patient Assessment & Care:
• Evaluate patient health status and medication needs.
• Develop and implement individualized medication plans.
• Monitor patient progress and response to treatment.
• Collaboration & Consultation:
• Work closely with physicians, nurses, and other providers.
• Provide expert drug information and recommendations.
• Participate in patient rounds with the healthcare team.
• Medication Management:
• Optimize medication orders and regimens.
• Ensure appropriate dosages, drug selection, and administration.
• Identify and resolve untreated health problems.
• Patient & Family Education:
• Educate patients on how to take medications, potential side effects, and adherence.
• Support patient understanding of overall health management (diet, exercise).
• Medication Safety & Quality Improvement:
• Reduce unnecessary drug use (e.g., antibiotics).
• Develop and implement clinical pharmacy programs.
• Maintain documentation and records.
• Operational Support (may vary):
• Support centralized/decentralized medication systems.
• Manage drug stock, storage, and supply (more general hospital pharmacy role).

Job Types: Full-time, Permanent

Pay: ₹20,000.00 - ₹25,000.00 per month
