· Conduct medication reconciliation on admission, during hospitalization, and at discharge to ensure accuracy and appropriateness.
· Review physician orders and prescriptions for correct drug selection, dosage, route, and frequency.
· Monitor patients for medication side effects, interactions, and adverse reactions, providing interventions as necessary.
· Perform therapeutic drug monitoring (TDM) for high-risk medications (e.g., anticoagulants, immunosuppressants).
· Review and update patient medication profiles and ensure proper documentation.
· Collaborate with multidisciplinary healthcare teams (doctors, nurses, dietitians, etc.) to optimize medication therapy and patient care.
· Provide evidence-based recommendations for drug therapy management.
· Participate in daily rounds and contribute to treatment decisions, especially regarding complex or high-risk medications.
· Offer advice on drug dosing, administration techniques, and alternative therapeutic options.
· Educate patients and their families on medication use, side effects, and drug safety.
· Provide guidance on the proper administration of medications and dietary interactions.
· Answer patient queries regarding their medications, including new medications, side effects, and potential drug interactions.
· Monitor clinical outcomes and medication effectiveness through regular follow-up assessments.
· Contribute to the development and implementation of hospital protocols, formularies, and treatment guidelines.
· Identify and report medication errors, adverse drug reactions, and other medication-related incidents, and assist in developing corrective measures.
· Support the hospital’s antimicrobial stewardship program by reviewing antibiotic use and contributing to infection control efforts.
· Train and mentor pharmacy students, residents, and other healthcare professionals on medication therapy management and safe prescribing practices.
· Participate in the development and delivery of in-service education sessions for hospital staff on topics related to pharmacotherapy and medication safety.
· Engage in clinical research and participate in hospital-based or external clinical trials.
· Review and contribute to the latest pharmaceutical and therapeutic literature to stay current on treatment protocols and drug developments.
· Participate in performance improvement initiatives to enhance the quality of care provided within the hospital.
· Ensure that all pharmacy practices and services comply with hospital, state, and federal regulations, including those from the Food and Drug Administration (FDA), Drug Enforcement Administration (DEA), and other relevant bodies.
· Maintain confidentiality of patient health information in accordance with HIPAA and hospital policies.
Any other duties assigned by management from time to time.
