Clinical Pharmacist - Ambulatory Care

Position Summary:

We are seeking an experienced and driven Clinical Pharmacist – Ambulatory Care to join our growing team. This role plays a key part in executing clinical service programs and supporting direct patient care initiatives. The Clinical Pharmacist (CP) will work under the direction of the Director of Clinical Services (DCS) and serve as a leader and implementation specialist within the clinical team.

Key Responsibilities:Clinical Services & Patient Care

  • Review and verify prescriptions from authorized prescribers
  • Provide comprehensive medication counseling, addressing side effects, interactions, dosage, and storage
  • Perform Drug Utilization Reviews (DUR)
  • Deliver direct patient care including Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Continuous Glucose Monitoring (CGM)
  • Provide Medication Therapy Management (MTM) services
  • Integrate into medical practice workflows and collaborate closely with providers

Collaborative Practice

  • Follow established collaborative practice agreements and protocols
  • Maintain professional relationships with internal and external stakeholders
  • Provide timely feedback to the DCS on clinical outcomes and provider engagement

Leadership & Administration

  • Manage and supervise staff pharmacists and clinical pharmacy technicians
  • Oversee KPI performance and assist in quality improvement initiatives
  • Provide coverage planning for team schedules and time-off requests
  • Contribute to the development and execution of clinical programs and business objectives

Qualifications:

  • Doctor of Pharmacy (PharmD) degree
  • Active Georgia pharmacist license
  • Post-graduate residency (PGY1) preferred or 2+ years of direct patient care experience in ambulatory care or similar setting
  • Immunization and MTM Certifications
  • BLS-CPR certified
  • Minimum 30 CPE hours every 2 years (20+ hours focused on chronic disease management)

  • Business acumen and understanding of regulatory compliance
  • Able to work independently and adapt to changing priorities

Physical Requirements:

  • Ability to stand for long periods
  • Lift and carry up to 25 pounds (occasionally up to 50 pounds)
  • Manual dexterity for routine pharmacy operations
  • Normal vision, speech, and hearing (corrected if necessary)

Job Type: Full-time

Pay: $120,640.00 - $128,960.00 per year
