The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The pharmacist is responsible for analyzing real and potential drug-related medical problems and

The primary function of a clinical pharmacist to assure the safe and appropriate use of medications and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; evaluation, dispensing and providing medications, and drug information. The pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies and procedures established at the national, VISN and local levels.
Functions of the Position
A. Clinical
i. Reviews patients' medication profiles and medical records as necessary for appropriate drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Can interpret routine laboratory values and to apply that knowledge in the evaluation of drug therapy and patient care. Communicates findings with prescribers and provides appropriate alternatives to current treatment regimens as needed before processing medication orders.
ii. Provides medication counseling to patients as needed and documents appropriately.
iii. Participates in multidisciplinary review of patients, evaluates pertinent laboratory data, drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.
iv. Reports adverse drug reactions (ADR), near misses, and medication errors in alignment with local policies and procedures.
v. Reviews and evaluates orders for non-formulary and restricted drugs for appropriateness and compliance based on established criteria and/or clinical
judgement.
vi. Has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings and local policy.
B. Dispensing
i. Reviews all medication orders for appropriateness, drug selection, dosage, and route of administration prior to dispensing.
ii. Ensures that all orders are filled in a timely manner.
iii. When appropriate fills and dispenses outpatients' prescriptions to veterans in the Durham VA Outpatient Pharmacy setting.
iv. Communicates to procurement staff to ensure adequate drug stock, inspects drug storage areas, supervises filling medication orders.
v. Ensures drugs dispensed are completely and accurately labeled and packaged in full compliance with Federal rules and regulations before verifying and releasing medication orders
vi. Maintains appropriate records and completes reports as necessary.
C. Education
i. Completes all required training in a timely manner.
ii. Responsible for maintaining his/her professional competency by keeping abreast of current medical and pharmaceutical literature, new drugs and therapies, and applies this knowledge in his/her daily duties.
iii. Manages various actions, as appropriate, for medication orders to include, but not limited to: pending orders, alerts and consults
iv. Serves as an effective preceptor/instructor for trainees (students, technicians, volunteers) conducting on-the-job training, orientation and in-services.
