Required Qualification
- Pharm D (Doctor of Pharmacy)
- Minimum 4–5 years of relevant hospital experience
We are looking for A Clinical Pharmacologist to join our team. Ideal candidate plays a crucial role in ensuring the safe, effective, and rational use of medicines within the hospital.
Key responsibilities include:
- Medication Safety & Monitoring
- Review and monitor prescriptions to prevent medication errors, interactions, and inappropriate dosages.
- Identify, document, and report Adverse Drug Reactions (ADRs) and near-miss events.
- Conduct medication audits and recommend corrective and preventive measures.
- Formulary Management
- Support the Pharmacy & Therapeutics (P&T) Committee in drug selection and review.
- Evaluate cost-effectiveness, efficacy, and safety of drugs.
- Clinical Support
- Provide therapeutic guidance to clinicians and nursing staff.
- Assist in dose adjustments for special populations (pediatric, geriatric, renal, and hepatic cases).
- Antimicrobial Stewardship (AMS)
- Collaborate with clinicians to ensure rational antibiotic use and monitor antimicrobial resistance.
- Actively participate in the Infection Control Committee, as mandated by NABH 6th Edition.
- Policy Development & Training
- Develop SOPs for medication management and high-alert drug handling.
- Conduct staff training on medication safety, ADR reporting, and rational drug use.
- Quality & Compliance
- Support hospital management in implementing NABH standards under MOM (Management of Medication) and PSQ (Patient Safety & Quality Improvement).
- Ensure documentation and evidence of compliance for accreditation audits.
