Overview
The Clinical Pharmacy & Medication Adherence Program Lead provides strategic, clinical, and operational leadership for pharmacy-driven medication management initiatives across the organization. This role is responsible for developing, governing, and continuously improving medication adherence, transitions-of-care, polypharmacy, and other targeted clinical pharmacy programs.
The Program Lead designs program infrastructure, establishes standardized processes, monitors performance, ensures regulatory and clinical quality compliance, and supports the clinical pharmacy teams delivering patient-level services. The role partners closely with clinical leadership, operations, analytics, and external stakeholders to ensure pharmacy programs are scalable, data-driven, and aligned with value-based care objectives. This position directly manages pharmacy team.

Qualifications
Licensure: Active pharmacist license in good standing (multi-state licensure preferred or willingness to obtain).
Experience:
Certifications (Preferred): Board Certification (e.g., BCPS, BCGP,BCACP), MTM certification, or project management training.

The working environment and physical requirements of the job include:
Work is performed indoors in a setting with air conditioning and artificial light. Travel to and work in offices or other environments is required.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
