Overview

The Clinical Pharmacy & Medication Adherence Program Lead provides strategic, clinical, and operational leadership for pharmacy-driven medication management initiatives across the organization. This role is responsible for developing, governing, and continuously improving medication adherence, transitions-of-care, polypharmacy, and other targeted clinical pharmacy programs.

The Program Lead designs program infrastructure, establishes standardized processes, monitors performance, ensures regulatory and clinical quality compliance, and supports the clinical pharmacy teams delivering patient-level services. The role partners closely with clinical leadership, operations, analytics, and external stakeholders to ensure pharmacy programs are scalable, data-driven, and aligned with value-based care objectives. This position directly manages pharmacy team.

Duties & Responsibilities

• Program Development & Governance
  • Design, implement, and oversee organization-wide clinical pharmacy and medication adherence programs.
  • Establish standardized workflows, protocols, documentation standards, and quality measures for pharmacy initiatives.
    • Ensure programs align with evidence-based guidelines, regulatory requirements, and organizational strategy.
    • Lead program expansion, enhancement, and innovation efforts in response to evolving patient and payer needs.
    • Anticipate emerging trends and proactively adjust strategies to maintain and enhance Star measure performance.

  • Operational Oversight & Performance Monitoring

    • Define and monitor key performance indicators (KPIs) related to medication adherence, safety, outcomes, and cost of care.
    • Partner with analytics and operations teams to develop dashboards, reports, and continuous improvement plans.
    • Identify operational gaps, risks, and opportunities; implement corrective actions and best practices with added focus on areas of greatest opportunity or risk.
    • Support readiness for audits, accreditation, and payer reporting requirements.

  • Clinical Pharmacy Team Management

    • Provide clinical and operational guidance to pharmacists and pharmacy staff delivering patient-level services.
    • Serve as a subject matter expert for medication management, adherence strategies, pharmacy related quality measures and pharmacy workflows.
    • Support training, onboarding, and ongoing competency development for clinical pharmacy teams.
    • Act as a resource for complex medication-related questions, escalation pathways, and program interpretation.
    • Provide direct patient support and conduct outreach to members, providers, and external partners to assist the clinical team in coordinating care and addressing medication-related needs as needed

  • Cross-Functional Collaboration

    • Collaborate with physicians, nurses, care managers, and operational leaders to integrate pharmacy programs into broader care models.
    • Partner with IT and informatics teams to optimize EHR tools, documentation, and pharmacy-related technology solutions.
    • Work with payer, quality, and contracting teams to support value-based arrangements and pharmacy-related initiatives.
      
    • Compliance, Quality, & Risk Management
      • Ensure pharmacy programs comply with state and federal regulations, payer requirements, and internal policies.
      • Support medication safety initiatives, quality improvement efforts, and risk mitigation strategies.
      • Stay current on pharmacy practice standards, regulatory changes, and industry trends.

Qualifications

• Education:Doctor of Pharmacy (PharmD) from an accredited school of pharmacy required.

  Licensure: Active pharmacist license in good standing (multi-state licensure preferred or willingness to obtain).

  Experience:

  • 5+ years of experience in clinical pharmacy, medication management, or population health.
  • Experience developing or leading pharmacy programs, quality initiatives, or operational workflows.
  • Deep knowledge of CMS Part D Stars methodology, pharmacy quality measures, and regulatory updates that impact performance.
  • Experience in value-based care, care management, or transitions-of-care programs preferred.
  • Experience in leading pharmacy teams is preferred.

  
  Certifications (Preferred): Board Certification (e.g., BCPS, BCGP,BCACP), MTM certification, or project management training.

  Technical Skills:

  • Experience with EHR systems, pharmacy documentation platforms, and data reporting tools.
  • Proficiency with Microsoft Office or similar productivity tools.

  Soft Skills:

  • Strong leadership, communication, and stakeholder engagement skills.
  • Ability to manage multiple initiatives and prioritize in a fast-paced environment.
  • Analytical mindset with a focus on quality improvement and outcomes.

The working environment and physical requirements of the job include:

Work is performed indoors in a setting with air conditioning and artificial light. Travel to and work in offices or other environments is required.

In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
