Clinical Professionals

We are seeking Clinical Professionals (RN, LPNs, ADNs, social workers, etc.) to work as Patient Care Coordinators with Chronic Care Patients in a Call Center environment
Bi-lingual in Spanish and English.
Will service patients by working on their care plan and phone summary provided by the caller. They will help build a detailed care plan for patients in order to be viewed by the physician and fill in all the gaps of the care plan so that the physician can recalibrate medication dosages, address patients’ needs and refer patients to specialists when needed.
Qualifications:

• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Knowledge of EMRs
