Clinical Quality Assurance

We are seeking an experienced and detail-oriented Registered Nurse (RN) to join our Home Health Quality Assurance team. The ideal candidate will have strong knowledge of home health regulations, OASIS documentation, Medicare guidelines, and clinical compliance standards.

This role is responsible for reviewing clinical documentation for accuracy, completeness, and regulatory compliance while supporting clinicians in maintaining high-quality patient care standards.

The RN Quality Assurance Nurse will work closely with field clinicians, intake staff, and leadership to ensure timely chart review, accurate coding support, and adherence to agency policies and CMS requirements.

Responsibilities

• Review home health clinical documentation for accuracy, completeness, and compliance
• Perform OASIS review and validation
• Ensure documentation meets Medicare, state, and agency standards
• Review plans of care, visit notes, recertifications, discharges, and physician orders
• Identify documentation deficiencies and communicate corrections to clinicians
• Assist with quality improvement initiatives and audit preparation
• Support compliance with CMS Conditions of Participation
• Collaborate with clinical staff to improve documentation quality and outcomes
• Monitor timeliness of chart completion and physician signatures
• Participate in staff education and training as needed
• Maintain confidentiality and HIPAA compliance at all times

QualificationsRequired

• Active and unrestricted RN license in Arizona
• Minimum 2 years of home health experience
• Strong knowledge of OASIS documentation
• Experience with Medicare and home health regulations
• Excellent attention to detail and organizational skills
• Strong communication and critical thinking abilities
• Proficient computer and EMR skills

Preferred

• Previous Quality Assurance or Clinical Review experience
• OASIS certification (COS-C or HCS-O)
• ICD-10 coding knowledge
• Experience with KanTime EHR system
• Case management or supervisory experience
