Job Overview
We are seeking a dynamic and experienced Clinical RN Consultant specializing in Skilled Nursing Facilities (SNF) to join our dedicated healthcare team. Responsibilities
- Provide comprehensive clinical consultation to SNF staff on patient assessment, care planning, and treatment protocols, including specialized areas such as memory care, hospice & palliative medicine, and Alzheimer's care.
- Assist with complex medical evaluations, including diagnostic evaluation and medication management, ensuring adherence to HIPAA regulations and medical documentation standards.
- Support facilities in implementing and maintaining compliance with industry standards such as NCQA standards and CMS regulations.
- Conduct training sessions on topics like aseptic technique, IV infusion, suturing, catheter care, infection control, and emergency procedures including airway management and ventilator use.
- Collaborate with interdisciplinary teams on discharge planning, case management, and quality improvement initiatives to optimize patient outcomes.
- Review medical records for accuracy using ICD-10 coding and CPT coding; ensure proper documentation of patient care activities.
- Provide guidance on specialized procedures such as dialysis access management, ostomy care, gastrostomy tube feeding, and laboratory specimen collection/processing.
Skills
- Extensive knowledge of EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation and data retrieval.
- Proficiency in managing diverse clinical scenarios including acute pain management, trauma medicine (Level I & II trauma centers), infectious disease care, and critical care environments like ICU or PICU.
- Strong experience with medical-surgical nursing, internal medicine, geriatrics—including long-term care facilities—and home & community care settings.
- Skilled in performing physical examinations, vital signs assessment, venipuncture (phlebotomy), IV insertion, catheterization techniques, and sterile processing procedures.
- Ability to triage patient needs effectively across urgent care settings or emergency room environments; adept at working with people with disabilities or developmental delays.
- Familiarity with medical imaging procedures such as sonography or mammography; understanding of laboratory experience for diagnostic evaluation.
- Excellent communication skills for teaching hospital experiences or outpatient clinics; capable of providing health coaching and education tailored to diverse populations including pediatric patients or elderly adults.
