Position Summary

The Clinical Supervisor – Home Health provides direct oversight and coordination of patient care delivery within the home health setting. This role is responsible for supervising clinical staff, ensuring care is delivered safely, effectively, and in compliance with agency, state, federal, and accreditation standards.

The Clinical Supervisor serves as a key operational and clinical resource supporting patient outcomes, staff performance, care coordination, and day-to-day workflow management across the Home Health and mobile care continuum.

Key Responsibilities

Clinical Leadership & Home Health Operations

  • Provide clinical supervision and coordination of Home Health patient care services
  • Ensure care delivery aligns with physician orders, individualized care plans, and agency policies
  • Supervise and support field clinicians to maintain competency, compliance, and quality patient care
  • Provide coaching, mentorship, and performance feedback to clinical staff
  • Serve as a clinical resource for escalated patient, caregiver, and staff concerns

Care Coordination & Documentation

  • Utilize electronic medical records (EMR) to coordinate and monitor patient care services
  • Review clinical documentation, including OASIS and visit notes, for accuracy, completeness, and timeliness
  • Support coordination of referrals, admissions, transfers, and discharges
  • Collaborate with interdisciplinary teams to ensure seamless patient transitions and continuity of care

Compliance & Quality Assurance

  • Help ensure compliance with Medicare Conditions of Participation and regulatory requirements
  • Participate in quality improvement initiatives, audits, and corrective action plans
  • Promote adherence to HIPAA, agency standards, and patient safety protocols
  • Support operational excellence and continuous improvement within Home Health services

Qualifications

Required

  • Current Registered Nurse (RN) license in the practicing state
  • Current CPR certification
  • Minimum of 2 years of Home Health experience
  • Strong knowledge of Home Health regulations, documentation, and patient care coordination

Preferred

  • 2+ years of clinical leadership, supervisory, or management experience
  • Experience supervising field-based or mobile care clinicians
  • Knowledge of OASIS documentation and Medicare compliance

Work Environment
