Job Overview
We are seeking a dedicated and experienced Nursing Supervisor to oversee the nursing as well as therapy staff and ensure high-quality patient care within our facility. The ideal candidate will possess a strong background in home health environment, experience as field nurse case manager. This role requires excellent leadership skills, a commitment to patient safety, and the ability to manage complex medical situations effectively.
Duties
- Supervise staff and oversee coordination of patient care activities to ensure adherence to best practices and regulatory standards.
- Conduct assessments of patient needs and develop individualized care plans in collaboration with healthcare teams.
- Ensure accurate medical documentation and compliance with California title-22 and with ICD-9 coding for billing purposes.
- Facilitate communication between patients, families, and healthcare providers to enhance patient satisfaction and outcomes.
- Provide training and support for nursing staff on laboratory procedures, medication administration, and emergency response protocols.
- Provide support to clinicians and back office staff for supply orders and follow ups.
- Monitor patient progress and adjust care plans as necessary to meet changing health needs.
Skills
- Proficient in home health practices and senior care methodologies.
- Excellent skills in patient care assessment and medical documentation.
- Familiarity with California Title-22 compliance requirements for home health.
- Familiarity with ICD-9 coding for accurate record keeping.
- Ability to work collaboratively within a multidisciplinary team while demonstrating strong leadership qualities.
- Effective communication skills to interact with patients, families, and healthcare professionals.
- Experience in home care settings is a must along with a solid understanding of care procedures.
- Comfortable with technological communication tools such as instant messaging, computer based phones and EMRs.

Pay: $75,000.00 - $110,000.00 per year
