Clinical Therapist & Case Manager (Hybrid) LMSW, LGPC, LCSW-C, LCPC

$2,000 Bonus Offered + 54 Free CEU Credits

Job Summary

The Clinical Therapist & Case Manager provides comprehensive mental health services, including psychotherapy, assessment, treatment planning, and case management. This role supports individuals experiencing mental health, emotional, behavioral, or substance use challenges by delivering therapeutic interventions and coordinating care across services and providers.

Key Responsibilities

Clinical Therapy Duties

  • Conduct biopsychosocial assessments and diagnostic evaluations
  • Develop individualized treatment plans based on client needs
  • Provide individual, group, and/or family therapy
  • Conduct in-person and virtual client sessions at various locations, requiring regular travel across multiple counties.
  • Apply evidence-based therapeutic approaches (e.g., CBT, DBT, trauma-informed care)
  • Monitor client progress and adjust treatment plans as needed
  • Maintain accurate and timely clinical documentation

Case Management Duties

  • Coordinate care with healthcare providers, social services, and community resources
  • Advocate for clients’ needs within systems of care
  • Conduct regular follow-ups and wellness checks
  • Develop discharge and aftercare plans

Crisis Intervention

  • Assess and respond to crisis situations, including risk of harm to self or others
  • Develop and implement safety plans
  • Provide immediate emotional support and stabilization

Compliance & Documentation

  • Maintain client records in accordance with HIPAA and organizational standards
  • Complete progress notes, treatment plans, and reports within required timeframes
  • Participate in audits, supervision, and case reviews

Collaboration

  • Work as part of a multidisciplinary team
  • Communicate effectively with psychiatrists, nurses, social workers, and external agencies
  • Participate in team meetings and case conferences

Required Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, or related field
  • Active or eligible licensure in the State of Maryland (e.g., LMSW, LCSW, LGPC, or LCPC)
  • Knowledge of mental health disorders, diagnostic criteria, and treatment modalities
  • Strong interpersonal, organizational, and communication skills

Pay: $70000.00 / year
