Job Description: Case Manager

Program: South Carolina Community Long Term Care (CLTC)

Position Summary

Prestige Case Management Services LLC is seeking a dedicated and compassionate Case Manager to provide comprehensive care coordination services under the South Carolina Community Long Term Care (CLTC) program. The Case Manager will support elderly and disabled individuals in maintaining independence by coordinating home- and community-based services in compliance with Medicaid waiver guidelines and SCDHHS regulations.

Key Responsibilities Assessment & Service Planning

  • Conduct comprehensive in-home assessments to evaluate client needs, risks, and eligibility for CLTC services.
  • Develop individualized, person-centered service plans that promote independence, safety, and quality of life.
  • Complete reassessments and revise service plans as client conditions or needs change.

Care Coordination

  • Arrange and authorize services including personal care, respite, adult day health, home-delivered meals, and durable medical equipment.
  • Collaborate with healthcare providers, family members, and community partners to ensure continuity of care.
  • Ensure services are delivered according to approved plans and Medicaid regulations.

Monitoring & Client Support

  • Perform routine home visits and follow-up contacts to monitor client progress and service effectiveness.
  • Identify potential risks and intervene to prevent health and safety concerns.
  • Provide education and support to clients and families regarding available resources and services.

Documentation & Compliance

  • Maintain accurate, timely, and compliant documentation in electronic case management systems.
  • Complete all required forms, service authorizations, and reports in accordance with SCDHHS and company standards.
  • Ensure readiness for audits and quality assurance reviews.

Advocacy & Crisis Intervention

  • Advocate for clients to ensure access to needed services and supports.
  • Respond to crises, including hospitalizations or changes in living arrangements.
  • Facilitate transitions of care when necessary.

Minimum Qualifications

  • Bachelor’s degree in Social Work, Human Services, Nursing, Psychology, or a related field.
  • Minimum of two (2) year of experience in case management, healthcare, or social services preferred.
  • Valid driver’s license and reliable transportation required.

Preferred Qualifications

  • Experience with South Carolina CLTC or other Medicaid waiver programs.
  • Strong knowledge of home- and community-based services (HCBS).
  • Case management certification (CCM, CDMS) or licensure (LMSW, RN) preferred.

Required Skills & Competencies

  • Strong assessment, organizational, and problem-solving skills.
  • Excellent verbal and written communication abilities.
  • Ability to manage a caseload independently and meet deadlines.
  • Proficiency with electronic health record (EHR) or case management systems.
  • High level of professionalism, empathy, and cultural competence.

Work Environment

  • Hybrid role involving fieldwork (client homes) and remote/administrative tasks.
  • Travel required within assigned service areas in South Carolina.
  • Flexible scheduling may be required based on client needs.

Physical Requirements

  • Ability to travel and perform home visits in various environments.
  • Ability to sit, stand, bend, and lift up to 20 pounds as needed.
  • Ability to operate a computer and mobile devices for documentation.

Reporting Structure

  • Reports to: Director of Case Management / Supervisor
  • Works collaboratively with interdisciplinary teams and service providers.
