Duties
- Deliver direct patient care by assisting with activities of daily living, including bathing, dressing, feeding, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor and record vital signs accurately, ensuring timely communication of patient status to the healthcare team
- Administer medications and treatments following established protocols while maintaining strict adherence to HIPAA and infection control standards
- Support patients with specialized needs such as Alzheimer's, dementia, or behavioral health conditions through compassionate engagement and tailored care plans
- Assist with medical procedures including catheter care, phlebotomy, and aseptic techniques in various settings such as hospitals, clinics, or long-term care facilities
- Document patient information meticulously using Electronic Health Record (EHR) systems like Epic, Cerner, or eClinicalWorks for seamless medical records management
- Collaborate effectively with interdisciplinary teams in high-pressure environments such as Level I Trauma Centers or ICU units to ensure optimal patient outcomes
Skills
- Proven experience working in critical care settings
- Familiarity with medical terminology, anatomy, physiology, and clinical procedures across acute and post-acute care environments
- Skilled in using EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate documentation and patient tracking
- Experience providing specialized dementia and memory care in assisted living or nursing home settings
- Compassionate approach to working with individuals with disabilities or developmental disabilities across home & community care settings
