• CNA/CMT/HHA License/Certification (Companions Don't Need This)
• Annual Physical**(Must Have)(Doctor stating you are well and have no limitations to work)
• PPD/Chest X-Ray**(Must Have)(Chest X-Ray is a One-Time ONLY event once the initial chest-ray is completed there is a annual TB Clearance Required)
• Criminal Background Check
• CPR****(Must Have)
• First Aide(If Applicable)
• License/State ID****(Must Have)
• Social Security Card****(Must Have)
• Passport, Green Card, or Work Permit(If Applicable)
