We’re looking for a dedicated and compassionate Certified Nursing Assistant (CNA) to join our Remote Patient Monitoring (RPM) team. In this role, you’ll play a vital part in improving patient outcomes by collecting vital signs and supporting virtual care at skilled nursing facilities. Job Description:
We are seeking a compassionate and reliable Certified Nursing Assistant (CNA) to join our growing Remote Patient Monitoring (RPM) team! In this role, you will visit one or more partner facilities to take vitals on residents enrolled in our RPM program and support virtual care visits.

Key Responsibilities:

• Take and record vitals on designated residents (list provided) enrolled in the RPM program
• Recognize critical vital sign ranges and report as needed
• Assist with telehealth visits using the provided tablet (with the Chronic Care Management team and providers)
• Complete related documentation and tasks as assigned
