Responsibilities
- Deliver direct patient care, including assistance with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts.
- Monitor and record vital signs, observe patient conditions, and report changes promptly to the nursing team.
- Assist with medical procedures such as catheter care, medication administration under supervision, and basic life support techniques.
- Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Support patients with specialized needs including dementia care, Alzheimer's care, memory care, behavioral health challenges, and working with individuals with disabilities or developmental disabilities.
- Collaborate with interdisciplinary teams in various settings such as hospitals (including ICU and trauma centers), long-term care facilities, assisted living communities, outpatient clinics, or home & community care environments.
- Adhere to aseptic techniques and infection control protocols during patient interactions to ensure safety for both patients and staff.
Skills
- Proven experience in critical care environments such as ICU, emergency medicine, or Level I & II trauma centers is highly desirable.
- Strong background in medical-surgical nursing, geriatrics, pediatrics, or post-acute care settings including hospice or home care.
- Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for accurate documentation.
- Knowledge of physiology and anatomy to effectively monitor patient health indicators like vital signs and physiological responses.
- Experience working with individuals with complex needs including infectious disease management and behavioral health support.
- Ability to perform phlebotomy procedures and assist with various medical procedures requiring aseptic technique.
