Job Overview
We are seeking experienced, compassionate and dedicated part time CNA (Certified Nursing Assistant), HHA (Home Health Aide), or medically trained caregiver to provide high-quality, in home care and support for adults with developmental disabilities and complex medical needs, The ideal candidate will possess a strong commitment to patient quality of life and well-being, excellent communication skills, and a thorough understanding of medical and personal care procedures within the scope of community supported living practice. Hours and available shifts will vary- this is a part time position with potential to advance to full time in future
Duties
- Assist with total care and activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts.
- Monitor and record vital signs , activity, intake and output while observing patient conditions for any changes or concerns.
- Administer medications and treatments following prescribed instructions
- Provide specialized care for patients with neurodevelopmental conditions priorotizing their health, safety, and comfort in a home environment
- Support patients with procedures such as feeding tubes, double verification of medications, wound management.
- Assist with meal preparation, feeding, laundry, light cleaning, and maintaining a clean and safe environment for clients.
- Collaborate effectively with Special Care Nursing team and to ensure comprehensive care plans and protocols are followed.
Experience and Requirements
- Min. 2 years Prior experience working with developmentally disabled and medically fragile adults in a home home health environment is highly desirable.
- Demonstrated proficiency in patient observation and documentation
- Knowledge of long-term care practices including skin assessment and protection, GT tube care and management, and medication administration.
- Familiarity with medical terminology and basic clinical procedures such as vital signs measurement, blood pressure, pulse oximeter, blood glucose monitoring.
- Proven ability to work well in a team environment in client home with peers.
- Certification as a CNA, HHA, MA, or equivalent healthcare certification is highly preferred; certification in CPR required.
- Must have reliable transportation and valid CA DL and pass livescan criminal background check
Pay: $24.00 - $26.00 per hour
