Responsibilities
- Provide direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Administer medications accurately according to prescribed orders, ensuring compliance with all safety protocols and documentation standards.
- Monitor vital signs and observe patients for any changes in condition, reporting findings promptly to nursing staff.
- Maintain detailed medical records and documentation within electronic health record (EHR) systems such as Point Click Care.
- Support specialized care needs including dementia, Alzheimer's, memory care, behavioral health, and hospice care by implementing individualized care plans.
- Assist with medical procedures such as catheter care, phlebotomy (blood draws), and aseptic techniques during treatments.
- Collaborate with multidisciplinary teams in diverse settings.
Skills
- Proven experience working with individuals with disabilities or in senior care environments such as nursing homes or assisted living facilities.
- Familiarity with EMR/EHR systems including Point Click Care, Matrix, or similar platforms for medical documentation.
- Strong knowledge of medical terminology, physiology, anatomy, and vital signs monitoring.
- Ability to perform medication administration accurately while adhering to safety protocols and HIPAA guidelines.
- Competence in using patient handling equipment like Hoyer lifts and understanding of geriatrics and dementia-specific care strategies.
- Excellent caregiving skills combined with a proactive attitude toward patient service and teamwork across various healthcare disciplines including pediatric (pediatrics) and behavioral health settings.
