Community Care - Case Manager

At Kept Inc, we help vulnerable adults avoid homelessness, exploitation and abuse while improving safety and overall wellness through care coordination and representative payee services. As part of our Financial and Community Connections program, most of our clients have stable housing and a team of professionals in place to meet their needs related to their primary disability. Our Account Managers provide ongoing monitoring of their wellbeing and care coordination to help them gain and maintain community support and maintain housing. However, for individuals who begin our services due to homelessness/risk of homelessness, exploitation, eviction intervention or other emergent circumstance, we provide Outreach and Community Care.

Our Outreach Navigator will work as a team member with Kept Inc's account managers, as well as, partnership providers in the community.

The tasks of the Outreach Navigator include but are not limited to:

• Delivery visits - dropping off supplies, payments, donations, etc
• Care coordination - prescription delivery, grocery delivery, deliver Tarc passes, deliver/pickup state supplement funding.
• Coordinate pickup/dropoff of housing items for individuals just moving into an apartment from homelessness.
• Virtual and In-Person Intakes
• Care Coordination at homeless shelters
• Attend partnership/community events
• Administrative duties: pickup/distribute mail, email, correspondence, complete payee accounting reports, maintain spreadsheets, online filing system, create social media posts, assist management, etc

The individual in this position will need to be comfortable coordinating scheduling and have flexibility during daytime business hours to complete tasks. Preferred to have at least some experience helping adults with disabilities.

This is a part-time grant funded position. Hourly position at $17.00/hour with a range of 20 hours per week with $250 Monthly Bonus and aprx. $200 Monthly Mileage Reimbursement for an average of $23,080 annually. Potential for growth after 6-9 months consistent employment meeting and/or exceeding expectations to increase pay per hour or increased hours per week.

Technology savviness, reliable transportation, trauma sensitive communication skills, compassion for others, problem solving skills, proficient written and verbal communication skills, open to learning, ability to self motivate and manage and adherence to an at home/flexible work schedule is required. Hire is contingent on passing criminal and abuse/neglect background checks.
