Position: Assertive Community Treatment (ACT) Case Manager

Location: Hays, Kansas | HPMHC

Pay: starting at $25 per hour | + Incentive Pay | $500 SIGN-ON BONUS!

Job Type: Full-time | In-person

Schedule: typical hours Monday-Friday 8am-5pm | On-call rotation

What You’ll Do:

• Provide community-based case management and rehabilitative services to adults with serious mental illness and co-occurring conditions.
• Deliver most services in clients’ homes and throughout the community to promote independence and recovery.
• Develop, implement, and maintain individualized service plans in collaboration with consumers, families, and treatment team members.
• Coordinate care with hospitals, medical providers, community partners, and other stakeholders to ensure continuity of care.
• Support clients in improving life domains including housing, employment, education, social relationships, health, and legal matters.
• Practice assertive outreach to reduce hospitalizations, increase housing stability, and improve overall quality of life.
• Provide mobile crisis response and participate in an after-hours on-call rotation.
• Transport clients as needed and respond to community-based emergencies.
• Complete timely documentation and actively participate in supervision and team meetings.

Requirements:

• Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
• Demonstrated ability to work effectively with individuals experiencing serious mental illness, functional impairments, and complex needs. Strong interpersonal, communication, and crisis intervention skills.
• Must pass KBI criminal history background check, DCF Child/Adult Abuse and Neglect Registry checks, professional background checks, and complete required state-approved training.
• Valid driver’s license with a driving record consistent with insurability (no substance-related incidents within the past five years).
• Ability to travel regularly (Center vehicle provided) and provide client transportation.
• Proficiency with computers and general office technology.
