Schedule:
Monday – Friday 9am-5pm
Principal Responsibilities:
Managing a caseload of up to 16 assigned individuals/families experiencing chronic homelessness with a concurrent diagnosis of substance use or mental health needs.
Engage in rapport building with participants and provide service from a “choice” perspective while utilizing participant-centered, harm reduction, and strengths-based approaches.
Provide case management services including assessment, referrals to community supports, and creation and monitoring of an individual action plan to accomplish participant identified housing and treatment goals.
Schedule and maintain consistent, meetings with assigned individuals to have structured, intentional conversations focused on obtaining housing and supportive service options.
Document case management services through detailed and thorough case notes in Ehana for each participant to include bi-weekly meetings, client and collateral contacts each month, as well as any significant events and/or interactions.
Update assessment and action plans in accordance with the CSP-CHI requirements using Ehana.
Maintain organized client files both hard copy and electronic in compliance with contract requirements.
Develop a network of social service providers and refer participants to appropriate services such as financial assistance, entitlements, addiction services, advocacy, employment assistance, financial counseling, rental assistance, deposits, Domestic Violence services, Mental Health services, etc. and assist with follow up for those referrals.

*Experience working with people from very diverse backgrounds
*Excellent communication and conflict resolution skills
*Ability to implement and utilize limit setting techniques and appropriate boundaries
*Ability to speak, read and write English, bi-lingual preferred
*Ability to pass CORI/background check
*Possess a valid Massachusetts driver's license and reliable transportation
