We are seeking a well-connected and motivated Community Outreach & Intake Liaison who understands the New Jersey DDD system and has experience working with Support Coordinators, families, and community partners. This role is ideal for someone who enjoys relationship-building and wants to play a key role in expanding access to quality services for individuals with developmental disabilities.

Key Responsibilities

• Build and maintain strong professional relationships with DDD Support Coordinators throughout New Jersey

• Actively promote Ameriwell Care’s services and increase referral activity

• Coordinate and manage the intake process for individuals entering services

• Serve as a primary point of contact for families, support coordinators, and referral sources

• Track outreach efforts, referral pipelines, and census growth

• Represent Ameriwell Care at professional meetings, networking events, and community engagements

• Support smooth onboarding and service launch for new individuals

Qualifications

• Experience in NJ DDD, healthcare, or home care services, preferably with knowledge of the New Jersey Division of Developmental Disabilities system

• Strong communication and relationship-building skills

• Organized with strong attention to detail

• Comfortable working both in the office and in the field

• Reliable transportation for travel within New Jersey
