Summary:
The purpose of this position is to provide health education through group and individual encounters and engage community members in a variety of settings on identified and emerging health issues in Cleveland County and related work as apparent or assigned. Work is performed and supervised under the Community Health Services Director.

Essential Functions and Responsibilities include:
" Primary responsibility to provide classroom instruction in reproductive health and safety using a state-approved, evidence-based curriculum to students, with active parent consents, enrolled in 9th grade health and physical education classes in Cleveland County Schools. Complete all requirements, including data collection using student/faculty/staff surveys, to ensure program delivery with fidelity. Support the work of the Teen Pregnancy Prevention Coalition and the Strong Teens Youth Council as advisors to this program.

" Facilitate classroom instruction on various health topics such as substance misuse, nutrition, physical activity and chronic disease prevention to students enrolled in Cleveland County Schools at all levels (elementary, intermediate, middle and high schools). Complete all requirements, including data collection using student/faculty/staff surveys, to ensure program delivery with fidelity.

" Participate in agency and community-sponsored coalitions addressing health topics as needed, including regular attendance at coalition/committee meetings and special events sponsored by the coalitions.

" Serve as a facilitator for various agencies and community-sponsored coalitions as needed, which includes determining agendas, providing communication and organization of meetings.

" Facilitate delivery of services such as the SOAR program to students who violate the substance abuse policies of Cleveland County Schools focusing on education and prevention delivered to individuals and/or groups. Obtain and maintain certification in the use of the Substance Abuse Subtle Screening Inventory (SASSI) as an assessment tool to determine the most appropriate strategies to engage students in substance misuse education and prevention.

" Research and develop patient-centered opportunities to increase access to healthy foods including but not limited to on-site garden beds, a prescription program to encourage healthy food choices and establishment of an on-site market for fresh fruits and vegetables for patients and employees.

" Assist in the delivery of the Med-South Lifestyle program and the T2 Diabetes Prevention program to targeted populations in the county to enhance education about healthy food choices.

" Serve as a facilitator for community engagement programming for youth and adults developed by the health education unit. Assist in marketing the programs to church and community organizations, assist in implementation and delivery of the programs. Assist in developing and implementing an evaluation component for each program delivered.

" Provide general support for all activities involving the health education/health promotion unit and the clinical areas of the Cleveland County Health Department as directed.

Education & Experience:
" Bachelor's Degree in health education, health promotion, public health or related health field or equivalent combination of education and experience. Certification as CHES preferred but not required.

Knowledge, Skills, and Abilities:
" Exhibit competency in carrying out planned health education programs; Considerable knowledge of community resources and services; Ability to organize and direct community activities; Ability to plan and conduct needs assessments; Ability to relate well to multiple population groups; Ability to express oneself clearly and effectively in oral and written form; Ability to carry out evaluation activities; Ability to manage multiple projects simultaneously with positive results.

Work Location:
This role must work from the location identified at the time of hire with work travel required as needed. Any change of work location, even on a short-term or interim basis, must be reviewed by the Supervisor and HR for approval.

In the event that the State Emergency Response Team is activated, this position will be required to participate if deemed necessary.
